By using the Site, requesting a free confidential consultation, requesting free insurance verification, or otherwise interacting with us, you agree to be bound by these Terms and our Privacy Policy, which is incorporated by reference.

2. No Medical Advice; Consultation Requests

The Site is provided for general informational purposes and does not constitute medical advice, a diagnosis, or a treatment relationship. Submitting a form or calling admissions does not create a provider-patient relationship. Appropriateness for any program is determined through our admissions and clinical process. We work with Private PPO plans and private pay. We are not able to accept Medicaid or Medicare at this time.

3. Eligibility

You must be at least 18 years old (or the age of majority in your jurisdiction) to submit information through the Site.

4. Communications

You may receive email and phone communications related to your inquiry, free consultation, or insurance verification. SMS and text messages are sent only if you check the optional SMS consent checkbox. Using this Site or providing a phone number does not constitute consent to receive text messages.

Grata House may send you SMS/text messages only if you affirmatively check the optional SMS consent checkbox. Providing a phone number or submitting a form without checking that box does not constitute consent. Using this website does not constitute SMS consent. Messages may include inquiry responses, free consultation scheduling, insurance verification follow-up, appointment confirmations, reminders, and admissions updates.
